Carmella Rose Health Foundation

Carmella Rose Health Foundation is a 501 (c)(3) organization that has been a pioneer in community navigation since 2004. Helping individuals navigate the complex healthcare system in Northeast Ohio is at the forefront of our mission. While working at the Redeemer Crisis Center, one of our patient navigators met Frank, a man who had just been notified through a blood sugar screening at the center that his blood glucose levels were in the danger zone and he should see a doctor immediately. Frank originally resisted, but after the encouragement and education provided by our navigator, he finally agreed. Later that evening, Frank called back to let us know he went to the Emergency Room and was diagnosed as a diabetic. Without the intervention of our navigator and the services we provided, Frank’s condition could have become much worse. Instead, Frank is a healthier and more informed man because of our help. Frank sincerely appreciated our help in assisting with his medical condition, despite his initial hesitation. There are thousands of individuals just like Frank that could be living with undiscovered conditions that can materialize into much more severe health problems.
