Naomi Daseul's Thumb Surgeries

One time in my life, I watched Nick Vujicic's videos and his testimonies and had imagined what it would be like living with such conditions forever. To be honest, someone like me who would label myself as a pastor had not even put much effort in understanding their needs and less inclined to sympathize surpassing the mere intake of the shallow inspirations.

Naomi was born on June 22, 2015. She was born in Vancouver, Canada as an almost full-term baby. There is a long story that my wife, Esther, can tell about her delivery, but from my side, it was a long moment of grief and guilt as a father, who should have things in control. As soon as I faced the challenges after meeting Naomi from the C-Section surgery room and in the Neonatal Intensive Care Unit, I could not give thanks to God for many hours. Sleepless nights continued while a massive load of information were dumped on our family from the doctors, nurses, geneticists, and social workers throughout the departments in the hospital. We have probably received hundreds of name cards from different physicians and nurses.


She is a very happy girl!!!
Naomi is diagnosed with VACTERL Association. This is almost like a syndrome, an association of multiple health issues in one person. Naomi had to go through a surgery to fix her imperforate anus on her first day of life. She had Ventricular Septal Defect (VSD) and Atrial Septal Defect (ASD) on her heart. The cardiologists have done a beautiful job on repairing her heart in her fourth month of age. Another one of our concern is her abnormal Tethered Cord, which is not a syndrome yet because her kidneys function well and she is very mobile on her feet. This is an area to watch while she grows. Lastly, Naomi has a hypoplastic left thumb. She is classified as having Thumb Hypoplasia Type IV (https://en.wikipedia.org/wiki/Thumb_hypoplasia). This is also known as floating thumb, which is most difficult to reconstruct even with advanced surgical skills.

We have consulted a plastic surgeon in Vancouver who wanted to perform index finger pollicization (making index finger to a thumb with having four-digits). We also visited a paediatric hand surgeon in Cleveland, United States, but we had received a similar answer. However, the doctor we met in the States referred us to another doctor in Japan, who would probably be an only person in the world to attempt a different method of surgery to reconstruct Naomi's thumb. We have been in contact with Dr Minoru Shibata in Niigata Central Hospital in Japan to accommodate Naomi's hand surgery in Mid 2017.

Details of the Surgery
The aim of the reconstruction is to convert from floating thumb to stable and mobile thumb with acceptable appearance and growing ability. However, it consists of tedious and complicated stages of operations.
1. First operation: Bone grafting and coverage with a vascularized pedicled flap. For the bone reconstruction of the thumb, 2nd toe proximal phalanx combining with iliac apophysis will be used. For the reconstruction of the donor toe phalanx, iliac bone with apophysis will be grafted. After bone grafting, the grafted bone needs to be covered with some vascularized skin. For this purpose, the doctor usually elevate a reverse posterior interosseous artery pedicled flap.
2. Second operation: Thumb opposition plasty (abduction plasty with transferring abductor digitiminimi muscle with its skin from the ipsilateral hypothenar area.
3. Third operation: Tendon transfer for mobilization of thumb (flexion, extension and adduction)
4. Fourth operation: Tenolysis, skin trimming, correction of bone alignment may be planned.



Duration
We are looking at a total of three or four surgeries with 3-5 months of intervals in between. The operation will take place in Japan and will require weeks of hospitalization. It may take about 9 to 15 months until Naomi is fully complete with her surgeries.
Cost
Since there is no possible coverage of health insurance for Canadians in Japan, we are facing substantial challenges. It costs about 750,000 JPY (about $8700.00 CAD) per surgery. Total cost is expected to be around $34800.00 CAD plus Flights and Accommodations.

Our Situation
We are a young family, relatively new in our marriage, that serves the Lord in a full-time ministry setting in the most desirable place on Earth. However, I think God is giving our family a second chance to consider another new life in Christ, to experience what it means to be followers of Christ through suffering and patience. Although we may not be the best tools for God's Kingdom, we learned to be sincerely humble before God and kneel for his grace to fall upon in the midst of such advanced and comfortable lifestyle in Canada. We named Naomi with a Korean name; Da [爹]-Seul [膝]. It means "to kneel before the Father." We pray that she will remind us to kneel down more before our sovereign Father, and we as parents, pray for Naomi to be the woman of prayer.
Esther and I are devoted to serving Christ forever, so too is our hope to see Naomi to be raised as a child of God who brings hope to the hopeless and joy to the joyless while given breath from God. We are very assured that God is guiding us in our steps and have shown his providence and unconditional love to make our journey up to here. We trust in the Lord, and through this campaign, we are confident in the people around us to guard our family to be true worshipers and pastors before God. Naomi's beautiful, complete, and well-functioning post-surgery hand will remind us each day of our lives how much blessed and privileged we are to have been loved to love others. 外科手術的細節
拇指重整之目的是在可以接受的外觀和生長能力下，將那隻浮動拇指轉變成為一隻概穩定又活動自如的拇指。可是整個程序就是一連串繁瑣和複雜的手術。
第一次手術：移植骨幹
第二次手術：移植肌肉

第四次手術：皮膚修整，亦可能會將骨幹對齊
所需時間
我們預計有三至四次的手術，每次相隔三至五個月。手術會在日本進行，並需要留院多星期。可能需要 9至15個月的時候 才能 完成全部的手術。

因為加拿大的醫療保險並不覆蓋日本地區，我們正面對龐大的挑戰。每次手術大約需要750,000日元(大約$8,7000加幣)。全部費用預計大約為$34,800加幣，再要加上機票和住宿費用。

我們是在全世界最好的地方全職服侍神的新婚年輕家庭。可是我想神讓我們的家庭有第二次機會去思量在基督裏的另一新生命，就是在受苦和忍耐當中，去經歷何謂基督的跟隨者。雖然我們不是神國裏最好的工具，但我們學懂了在神面前誠心謙卑，在加拿大既先進又舒適的生活方式當中，跪下來讓神的恩典降臨在我們身上。 我們替Naomi改了一個韓國名字：Da [爹]-Seul [膝]，意思即是在天父面前跪下來。我們祈求她會提醒我們更多在大君王父神面前跪下來，和我們身為父母的，為Naomi祈禱，叫她成為一個以祈禱為重的人。

한번은 닉 부이치치씨의 간증 동영상을 보면서 상상을 해보았습니다. 저는 하나님 일을 하는 사역자임에도 불구하고 여전히 제 삶에 실제로 다가오지 않았던 기억이 납니다. 감동은 있었으나 저에게 변화를 준 것은 우리 딸 다슬 (Naomi)가 태어난 이후였습니다.
다슬이는 2015년 6월 22일에 캐나다 밴쿠버에서 태어났습니다. 제 아내에게 설명을 부탁하면 출산에 대해 아주 긴 이야기를 들을 수 있지만 모든것을 책임져야하는 아버지 입장에서 다슬이의 출산은 참 고민과 죄책감도 많았습니다. 우리 다슬이를 C-Section 수술실과 신생아 중환자실에서 만났을 때부터 시작된 어려움들은 만물의 창조주이며 제가 아바 아버지라고 고백했던 하나님께 감사하다는 표현이 나오기 까지 몇 시간이나 걸렸습니다. 그 이후 엄청난 숫자의 의사들과 간호사 선생님들을 계속해서 만나게 되었습니다. 다슬이를 위해 정기적으로 집에 방문해주는 간호사들도 많았습니다.

다슬이는 바테르 (VACTERL) 증후군으로 병원에서 판정합니다. 바테르 증후군은 Vertebral (척추) 기형, Anal (항문) 폐쇄, Cardiac (심장) 기형, 식도 폐쇄를 동반한 T-E (식도-기관) 이상, Radial (요골) 기형 혹은 Renal (신장) 기형이 동시에 3개 이상 ( 혹은 2개 이상) 으로 복합적으로 나타나는 것이 우연의 일치보다 더 많은 빈도로 일어나기 때문에 각 이상기관의 첫 자를 인용하여 VACTERL 연합이라고 부릅니다. 10만명의 생존 신생아 당 16례 정도 발생한다고 합니다. 다슬이의 경우는 태어나서 항문 폐쇄로 인해 첫날 수술을 하였고요, 심장에 구멍이 두개나 있어 4개월째 넘어서서 심장수술도 있습니다. 척추에 기형을 보이고 있으나 척추기형으로 인해 현재는 걸음걸이나 신장에 문제가 없으므로 계속해서 지켜보고 기도해주셔야 할 부분입니다. 나머지 하나는 다슬이의 왼쪽 엄지손가락의 기형입니다. Hypoplastic Thumb이라고 부르고 엄지손가락에 뼈가 없이 태어났습니다. (https://en.wikipedia.org/wiki/Thumb_hypoplasia). 손가락 기형 중에 성형하기 가장 어려운 Type IV입니다.
이 문제로 밴쿠버에 있는 성형외과를 통해 엄지화 수술 (검지 손가락을 엄지로 만드는 수술)을 추천 받았고 인터넷에서 논문을 보고 수소문하여 찾아간 미국 클리브랜드에 전문 의사선생님을 통해서도 동일한 대답을 얻었습니다. 그러나 미국에서 만난 의사 선생님이 전 세계에서 다슬이 손가락을 고쳐줄 의사가 있다면 한 명이 있을 거 같은데 소개를 해주겠다고 해서 일본 니가타 중앙병원에 계신 Dr. Minoru Shibata선생님을 소개 받았습니다. 그래서 올 2017년 중반에 방문과 수술일정을 계획하고 있습니다.
수술안내
수술의 목적은 현재 보유하고 있는 엄지손가락을 보존하여 정상적으로 움직이고 성장하면서 모양도 만들어주지만 몇차례 어려운 수술과정을 겪어야 합니다.
1. 뼈 이식 수술 (검지발가락), 피부이식 수술
2. 근육, 피부 이식
3. 힘줄 이식 (손가락이 움직이도록)
4. 힘줄박리술, 마지막 조율 및 피부성형
수술기간 Duration
총 3-4번의 수술을 필요로 하고 수술마다 3-5개월의 안정기간이 필요합니다. 수술은 일본 니가타 대학병원에서 하고 9개월에서 15개월안에 모든 수술일정이 마무리됩니다.
비용 Cost
저희 가족은 캐나다 시민권자입니다. 일본에서 하는 수술은 의료보험 해택을 받을 수 없어서 재정적으로 어려움이 있습니다. 대략 캐나다 달러로 $8700 정도가 한번 수술비용이고 총 $34800.00 수술비용과 추가적으로 비행기 및 숙박비용이 들게됩니다.

한번은 닉 부이치치씨의 간증 동영상을 보면서 상상을 해보았습니다. 저는 하나님 일을 하는 사역자임에도 불구하고 여전히 제 삶에 실제로 다가오지 않았던 기억이 납니다. 감동은 있었으나 저에게 변화를 준 것은 우리 딸 다슬 (Naomi)가 태어난 이후였습니다.
다슬이는 2015년 6월 22일에 캐나다 밴쿠버에서 태어났습니다. 제 아내에게 설명을 부탁하면 출산에 대해 아주 긴 이야기를 들을 수 있지만 모든것을 책임져야하는 아버지 입장에서 다슬이의 출산은 참 고민과 죄책감도 많았습니다. 우리 다슬이를 C-Section 수술실과 신생아 중환자실에서 만났을 때부터 시작된 어려움들은 만물의 창조주이며 제가 아바 아버지라고 고백했던 하나님께 감사하다는 표현이 나오기 까지 몇 시간이나 걸렸습니다. 그 이후 엄청난 숫자의 의사들과 간호사 선생님들을 계속해서 만나게 되었습니다. 다슬이를 위해 정기적으로 집에 방문해주는 간호사들도 많았습니다.

다슬이는 바테르 (VACTERL) 증후군으로 병원에서 판정합니다. 바테르 증후군은 Vertebral (척추) 기형, Anal (항문) 폐쇄, Cardiac (심장) 기형, 식도 폐쇄를 동반한 T-E (식도-기관) 이상, Radial (요골) 기형 혹은 Renal (신장) 기형이 동시에 3개 이상 ( 혹은 2개 이상) 으로 복합적으로 나타나는 것이 우연의 일치보다 더 많은 빈도로 일어나기 때문에 각 이상기관의 첫 자를 인용하여 VACTERL 연합이라고 부릅니다. 10만명의 생존 신생아 당 16례 정도 발생한다고 합니다. 다슬이의 경우는 태어나서 항문 폐쇄로 인해 첫날 수술을 하였고요, 심장에 구멍이 두개나 있어 4개월째 넘어서서 심장수술도 있습니다. 척추에 기형을 보이고 있으나 척추기형으로 인해 현재는 걸음걸이나 신장에 문제가 없으므로 계속해서 지켜보고 기도해주셔야 할 부분입니다. 나머지 하나는 다슬이의 왼쪽 엄지손가락의 기형입니다. Hypoplastic Thumb이라고 부르고 엄지손가락에 뼈가 없이 태어났습니다. (https://en.wikipedia.org/wiki/Thumb_hypoplasia). 손가락 기형 중에 성형하기 가장 어려운 Type IV입니다.
이 문제로 밴쿠버에 있는 성형외과를 통해 엄지화 수술 (검지 손가락을 엄지로 만드는 수술)을 추천 받았고 인터넷에서 논문을 보고 수소문하여 찾아간 미국 클리브랜드에 전문 의사선생님을 통해서도 동일한 대답을 얻었습니다. 그러나 미국에서 만난 의사 선생님이 전 세계에서 다슬이 손가락을 고쳐줄 의사가 있다면 한 명이 있을 거 같은데 소개를 해주겠다고 해서 일본 니가타 중앙병원에 계신 Dr. Minoru Shibata선생님을 소개 받았습니다. 그래서 올 2017년 중반에 방문과 수술일정을 계획하고 있습니다.
수술안내
수술의 목적은 현재 보유하고 있는 엄지손가락을 보존하여 정상적으로 움직이고 성장하면서 모양도 만들어주지만 몇차례 어려운 수술과정을 겪어야 합니다.
1. 뼈 이식 수술 (검지발가락), 피부이식 수술
2. 근육, 피부 이식
3. 힘줄 이식 (손가락이 움직이도록)
4. 힘줄박리술, 마지막 조율 및 피부성형
수술기간 Duration
총 3-4번의 수술을 필요로 하고 수술마다 3-5개월의 안정기간이 필요합니다. 수술은 일본 니가타 대학병원에서 하고 9개월에서 15개월안에 모든 수술일정이 마무리됩니다.
비용 Cost
저희 가족은 캐나다 시민권자입니다. 일본에서 하는 수술은 의료보험 해택을 받을 수 없어서 재정적으로 어려움이 있습니다. 대략 캐나다 달러로 $8700 정도가 한번 수술비용이고 총 $34800.00 수술비용과 추가적으로 비행기 및 숙박비용이 들게됩니다.

현재 상황 Our Situation
저희는 결혼 3년차에 들어간 젊은 부부이고 현재 밴쿠버 지역에 2세들을 섬기는 교회에서 전임으로 교회사역중에 임해있습니다. 하나님 주신 선물인 다슬이를 통해서 그리스도안에 새로운 삶의 기회를 주시는 듯 합니다. 하나님 나라에 최고의 도구는 아닐 지 몰라도 예수님을 따르는 것이 무엇인지, 예수님의 고난과 인내에 대해 조금이나마 알아가게 하시니 너무 감사드립니다. 캐나다라는 너무 좋은 환경에서 최고의 것들을 누리며 살던 저희들에게 하나님이 다슬이라는 딸을 주셨습니다. 아버지 다 [爹] 에 무릎 슬 [膝]이라는 이름을 쓰는 다슬이를 통해 창조주 하나님께 무릎꿇게 하시고, 저희 딸이 동일하게 기도의 사람으로 자라면 좋겠습니다. 그래서 소망이 없는 이들에게 소망을 주고 기쁨이 없는 이들에게 기쁨을 주는 하나님의 자녀 되게 기도 부탁드립니다. 주위에 계신 여러분들이 저희 가정을 보호해 주셔서, 참되게 그리스도의 종이 되고 예배자들이 될 수 있도록 지켜주실거라 또한 믿습니다. 긴 글 읽어 주셔서 감사합니다.
