The Diabetes Management Program will provide health screenings for individuals who might have diabetes, will follow treatment for those who are diagnosed and do not have ready access to healthcare providers due to transportation or other challenges. The Diabetes Management Program will also host an annual diabetes conference for those living with diabetes and their caregivers. It will also provide up-to-date information for healthcare providers.
