Need help getting a new wheelchair and access ramp

I’m 62 year old male that has a back problem and was bedridden for many months and my old wheelchair is worn out the tires come off the wheels every time I use it . At the present time I have no income due to workman's comp check being stopped , and ssdi has not been determined yet ,it's really scary riding in the old chair afraid going outside of the house and it breaks how will I get home is running through my mind ,or will I risk further injury if it happens to break . And any funds leftover after the wheel chair purchase will be put to the construction of a much needed ramp to help me get in and out of my residence.
