Please Help Me If You Can

Back in March i started having Chest Pain, Leg Pain, and Shortness of Breath. I was going back and forth to the Doctor. She was treating me for Asthma. I kept saying it's not Asthma. I know the difference. She wasn't listening even though i was telling her what's wrong. She sends me to a Pulmonologist and he didn't listen to nothing at all. That was May 2. By this point i had been sick 6 weeks. That Saturday a friend asked me what was going on and i told her. She said maybe i should see one of the Doctors she works for. Next day i tried to do some Laundry and almost has to crawl back to my room because i was so out of breath. My Friend called me the next day and made me an appointment because her Boss wanted me to come in right away. I went in at 4:30. He actually listened to me. He scheduled me for a CT of The Chest Angiogram for the next day. I went in and the next thing i hear is the Lady yelling find her brother because she's not going home. I was diagnosed with Saddle Pulmonary Embolism. Basically a Blood Clot that sits at the main Arteries to My Heart and Lungs. Doctors couldn't believe i was alive. It's the worst kind you can have and only 1-3% people survive that when caught early. It left so much scarring to my heart that my heart looks like that of someone that has had a heart attack and one of my Lungs partially Collapsed. I've been on 24hr Oxygen since May. I had to give myself Shots in my stomach twice a day which left me so sore and bruised because i can only give them in certain areas. I have to take 16 pills a day. I see the Oncologist twice a week to have my blood checked. My Short Term Disability from work has ran out October 14, 2018. my Long Term Claim was opened September 18, 2018. The Insurance company keeps finding ways to extend time to make a decision to approve me for Long Term Disability. My Primary Care Physician, My Pulmonologist, My Oncologist, and My Cardiologist have all told them that I can't work. They have requested all My Medical Records but kept telling me they haven't received them. I called to follow up myself and come to find out they haven't been paying for them. I know times are hard for everyone and I'm just asking you to help if you can. I'm trying to pay My Rent and Utilities. I don't have anywhere for me and My Daughter to go if we are evicted. Please help.
