Follow Me Home's first round of care package fundraising provided the funds Gilead Community Services needed to build and infrastructure to deliver case management and other community-based mental health care services to those most in need.
What began with several passionate community case managers gathering supplies and crafts has now grown into a weekly operation to deploy more human-centered healthcare during a public health crisis - a crisis not experienced universally by all Americans. The care package delivery service now supports a weekly in-person case management system to over 60 community members living in congregate care settings.
While much of the economy cautiously looks towards a re-opening, it's important for all of us Americans not to forget the lessons learned and the ones we continue to learn throughout the crisis. Not least of which: the crisis is not experienced in the same way by everybody. While renewed calls for increased telehealth services are needed, we also must understand that those at risk living in isolated, congregate care settings will continue to need in-person case management and connection.
Our new goal of $2,400 will provide us with the opportunity to continue supporting the care package delivery services and human-centered case management throughout the South-Central Connecticut region (a region whose congregate care settings have been hit particularly hard by the pandemic).
