Facing Homelessness with Stage 4 Kidney Disease

I Migdalia Cruz am in my early fifties and have advance stage 4 kidney disease.
I also have diabetes, high blood pressure and recently had a stroke.
I stay alive by using a peritoneal dialysis machine at home that I attach through my stomach.
My husband makes only $11 an hour.
We fell seriously behind on rent and bills and are facing eviction.
The landlord has been very kind and carried us for the last 2 years while we pay what we can. She does need to live too and needs payment towards our arrearage of approximately $20,000.
Any and all help will be sincerely appreciated since being homeless and on dialysis is a literal death sentence.
