Help Colleen come home from hospital!

Hi my name is Holli and I am fundraising for my mom Colleen Tapley. She has now been in the hospital for almost 3 weeks and we are trying to arrange to get her home but the house is not suitable for her health conditions and we are trying to set up a chair lift on the stairs to allow her to get around the house. As of now she has trouble just getting around the hospital bed and if we can't get the money together for her to have a chair lift for the house she will have to be admitted to another physical rehab facility where she will have to stay. Her physical and now mental health is not well and I really feel she would be better off at home to live out whatever time she has left with her current medical conditions. Any help that anyone could give would be greatful. Anyone that knows my mom already knows she is a loving and caring person who needs to be with her family not nurses and doctors.
