Help Jonathon Cheatle Avoid Homelessness

My name is Jonathon Cheatle. Some of you may know me from high school, college, gaming, or the West Coast. I am in need of emergency moving funds so that I do not become homeless, I have roughly two weeks before I have to move out. I put $5,000 down, but the reality is that just half of this after fees would do.
Why am I asking for help?
I moved out to Oregon to be with my girlfriend; however, after dealing with sickness and the stress of her going back to school, we have split up. This put me in a situation where I had to move almost immediately, and where I have lost all income (remote work) due to an unstable environment. To make the situation worse, I was hit by a car recently and am dealing with a hurt leg. Losing a partner, my income, my house, and getting hurt in the span of a month has been jarring, shocking, exhausting, and devastating.
I am diabetic and also on expensive medication for autoimmune disorders. Having these conditions is expensive and prohibitive, and without a residence, it's going to be very hard to take care of myself short term.
If I can get enough money to rent/sublet a place, I will be able to get back to generating some income and give myself a greater chance of not slipping into full homelessness. I don't mind explaining my situation further or answering questions.
