Help me catch up on finances

I never wanted to write this but at this point I need to. For the last year, I have had severe nerve pain. Now, some of it was expected as I have nerve damage in my right arm due to a vaccine injury. But I started getting the same pain everywhere. I went and in January was diagnosed with Chronic Inflammatory Demylenating PolyNeuropathy(CIDP). Due to that, I have not worked a full schedule since then. Then the falls started. Falling for no reason. Just losing control of my legs. So I was put out of work for PT. Then my brother killed himself. Massive depression hit. Doctors office refusing to sign needed paperwork as doctor went away for a month. To get my PCP to do it instead of the neurologist will be $75. Just a whirlwind of sadness. I tried to go out to do Uber. But the car has suddenly started to shut down on me. So it’s not safe to drive people in it. So I have no income. No money for rent. No money to get the paperwork filled out.
