Help Melissa Mahler after ADA claim was denied.

Let me preface this with I am reaching out to all the assistance programs I can.
My ADA extension claim was denied last week.
As some of you may know I have been out of work since mid July. I have been on medical leave while doctors have gone through diagnostics and I was finally diagnosed with Inappropriate Sinus Tachycardia and POTS. In layman's terms my resting heart rate is very high for no reason and gets even worse with any type of movement.
It took two months to diagnose and another month to finally get the medication I needed. Now that I have the medication I can start the physical therapy that my doctors have ordered.
My workplace has approved my extended medical leave but the absence management company has denied my ADA extension claim. I have not had any income since August. I have requested to file an appeal but that can take up to two months and it can still be denied again.
I am at a loss and need help. I am in no condition to return to work yet. Dealing with this medical issue on top of my everyday chronic health issues has been exhausting and debilitating.
I live alone and I am the sole earner in my household. I have no family to rely on for this amount of help.
At this point in time I have no feasible way to pay my future bills and I have already incurred late fees from missed payments.
I appreciate any help that is given. Thank you
