Launch Janie Appleseed MyPHD Pilot

These funds will be used to introduce new technology that helps families manage their personal health data (PHD) so they can stay healthy and enjoy life-long well-being. It's called MyPHD Wellness Manager.
The technology helps you and your family members create a "care plan" for yourselves. The plan includes care goals and activities recommended by your healthcare providers, as well as other goals and activities you've planned for yourself to address wellness in a holistic way. The technology helps you monitor progress toward your goals, and it keeps track of your health history as it unfolds.
The funds will be used to pilot Janie Appleseed MyPHD Wellness Manager with 120 families in Rhode Island over a one-year period. The pilot will include 20 families in each of these categories:
1. families taking care of an elderly parent
2. families taking care of a special needs child
3. families with a member living with a chronic condition
4. families with a member struggling with opiod addition
5. families taking care of a new baby
6. families with no particular health issues, and wants to stay healthy.
The pilot provides the education and support needed to get families up and running on MyPHD. It teaches families how to use the system and how to share information with care providers and other family members who are part of their care team.
The result of the pilot will be a technology needs assessment to identify areas where the technology meets consumer needs and areas where improvement is needed. Experience will be gained to assess demand for consumer-controlled technology to manage personal health data. The value propostion for using this type of technology will be clarified and quantified.
Your donations will be used to cover system configuration and development, subscription fees for all family participants, development and delivery of educational programs, running of participant feedback sessions, and creation of the pilot report to document all findings, conclusions and recommendations, and informational presentations to share learnings from the pilot experience.
Donors giving $500 or more will recieve a free one-year family subscription to MyPHD following the pilot. Donors giving $5,000 or more will be invited to participate in quarterly webinars discussing the progress and learnings from the pilot, as the experience unfolds. Donors giving $25,000 or more will recieve a copy of the needs assessment documentation and the final report about the experience gained from doing the pilot.
