Mein Name ist Lukas Ortmann. Ich bin 6 Jahre alt und möchte Sie in einem besonderen Anliegen um Ihre Unterstützung bitten. Um mir in meiner körperlich motorischen Entwicklung bestmögliche Chancen einzuräumen, hat sich meine Mutter um die Behandlungsmöglichkeit der SDR bei Dr. TS Park am Children’s Hospital in St. Louis, Missouri, USA bemüht.

My name is Lukas Ortmann. I am 6 years old and would like to ask for your support in a special matter. In order to give me the best possible chances in my physical and motoric development, my mother sought Dr. TS Park at Children's Hospital in St. Louis, Missouri, USA for potential treatment.

Ich wurde am 21. März 2017 als extremes Frühchen geboren und leide in Folge meiner Frühgeburt an einer irreparablen bilateralen spastischen Zerebralparese (GMFCS IV), Mikrozephalus, Dystrophie, Sprachentwicklungsstörungen sowie einer globalen Entwicklungsstörung.

Dennoch bin ich ein fröhlicher, lebensbejahender Kämpfer mit Entdeckergeist. Durch die intensive und unermüdliche Unterstützung meiner Mutter und die diversen Institutionen in Deutschland und der Slowakei habe ich es geschafft, in den vergangenen Jahren und Monaten unerwartete Fortschritte zu erreichen, die glücklicherweise fernab von ursprünglichen Prognosen sind. Entgegen der Erwartungen habe ich es erlernt, mich auf dem Boden zu drehen, in den Sitz zu kommen und am Walker zu gehen.

I was born extremely premature on March 21, 2017 and as a result of my premature birth I suffer from irreparable bilateral spastic cerebral palsy (GMFCS IV), microcephaly, dystrophy, language development disorders and a global developmental disorder. Nevertheless, I am a happy, life-affirming fighter with a spirit of discovery. Thanks to the intensive and tireless support of my mother and the various institutions in Germany and Slovakia, I have managed to make unexpected progress over the past few years and months, contradicting my initial prognosis.

Neben den vielen Terminen bei Kinderarzt, SPZ, Logopädie, Orthopädie, Hilfsmittelversorgern, Augenarzt, Physiotherapie (3-4 x pro Woche), Ergotherapie, PETÖ, Hippotherapie, Hyperbare Sauerstoffkammer (HBOT) sowie Heilpraktikern trainiere ich täglich physiotherapeutisch (Vojta, Bobath sowie mit dem Galileo Vibrationstraining). Gern nutze ich ebenfalls meinen Walker sowie das Therapiedreirad mit viel Begeisterung.

In addition to the many appointments with the pediatrician, SPZ, speech therapy, orthopedics, suppliers of medical aids, ophthalmologist, physiotherapy (3-4 times a week), ergotherapy, PETÖ, hippotherapy, hyperbaric oxygen chamber (HBOT) and naturopaths, I train daily in physiotherapy (Vojta, Bobath and with the Galileo vibration training). I also enjoy using my walker and the therapy tricycle.

Das Therapiereiten, die Teilnahme am Musikgarten sowie die Besuche im Schwimmbad entsprechen weiterhin meiner Leidenschaft für Bewegung und Musik. Im Kindergarten ist die soziale Integration mit Regelkindern meine größte Freude. Eine Einladung am Nachmittag bei meinen Freunden macht mich unendlich glücklich!

Hippotherapy, participation in music classes and visits to the swimming pool continue to reflect my passion for movement, animals and music. In kindergarten, the social integration with 'normal' children is my greatest joy. An invitation to one of my friends' houses in the afternoon makes me infinitely happy!

Ich bin so stolz auf die vielen kleinen und großen Fertigkeiten, die ich allein in diesem Jahr entwickelt habe.

Die unkontrollierte Spastik insbesondere in meinen unteren Extremitäten verhindert die gezielte Ansteuerung und das Trainieren bestimmter Muskeln. Eine Selektive Dorsale Rhizotomie (SDR) gibt mir die große Hoffnung, die nächsten Schritte für meine Entwicklung anbahnen zu können. Ich möchte so gern laufen, ohne ständig zu stolpern, und sicher stehen. Mein größter Wunsch ist es, mit meinen Freunden Fußball zu spielen und nicht immer vom Spielfeldrand im Rollstohl zuzuschauen.

The uncontrolled spasticity, especially in my lower extremities, prevents the directed control and training of certain muscles. SDR gives me great hope of being able to initiate the next steps in my development. I really want to be able to walk without tripping all the time and to stand securely. My biggest wish is to play football with my friends and not always watch from the sidelines in my wheelchair.

Die Selektive Dorsale Rhizotomie (SDR) ist ein neurochirurgisches, operatives Verfahren. Ziel dieses Verfahrens ist Spastikreduktion des zerebralen Ursprungs.

Selective Dorsal Rhizotomy (SDR) is a neurosurgical surgical procedure aiming at the reduction of spasticity with cerebral origin.

Wenn die willkürliche Bewegung der unteren Extremitäten durch Spastik eingeschränkt wird, wird auch gleichzeitig die Kräftigung der Muskulatur in diesem Bereich sowie das Erlernen von Motorik und die Ausprägung des Gleichgewichtssinnes erschwert.

Während der Selektiven Dorsalen Rhizotomie kommt es zu einem minimalen Zugang zur Wirbelsäule, um einen Zugang zu den Nerven der unteren Körperhälfte am Rückenmark zu erhalten. Mit Hilfe elektrischer Stimulation werden die Nervenbündel, die die Spastizität verursachen, identifiziert. Die abnormalen Nervenbündel werden selektiv durchtrennt und die normalen Nervenbündel nicht berührt. Dadurch werden die Informationen des Muskels an das Gehirn reduziert, was wiederum die Aktivität der Nervenzellen im Rückenmark ins Gleichgewicht bringt und somit die Spastizität eliminiert.

If the voluntary movement of the lower extremities is restricted by spasticity, it also makes it more difficult to strengthen the respective muscles and to develop motor skills as well as a natural sense of balance. During SDR, minimal access is made to the spine to access the lower body nerves at the spinal cord. With the help of electrical stimulation, the nerve bundles that cause spasticity are identified and cut selectively while the normal nerve bundles remain untouched. The subsequent reduced information from the muscle to the brain balances the activity of the nerve cells in the spinal cord, eliminating spasticity.

Bis heute ist die SDR-Operation das einzige Mittel, um Spastizität bei Kindern mit Zerebralparese dauerhaft zu reduzieren. Die SDR-Operation verbessert erwiesenermaßen das Erlernen willkürlicher Bewegung und erlaubt den Erhalt gesunder Gelenk- und Muskelstrukturen.

To date, SDR is the only means of permanently reducing spasticity in children with cerebral palsy. SDR has improved the ability to learn voluntary movement and to maintain healthy joint and muscle structures.

Dr. TS Park ist ein weltweit anerkannter Neurochirurg, der Vorreiter auf dem Gebiet der SDR ist und mit seiner minimalinvasiven SDR Methode bereits über 5.000 Patienten erfolgreich zu mehr Lebensqualität, Gesundheitsprävention sowie Unabhängig verhelfen konnte. Nachdem mir bereits in Deutschland Erfolgsaussichten nach einer Selektiven Dorsalen Rhizotomie (SDR) genannt worden sind, hat meine Mutter den umfangreichen Screening Prozeß für eine Behandlung durch Dr. TS Park in die Wege geleitet. Insbesondere seine minimalinvasive Herangehensweise, umfangreiche Erfahrung, medizinische Vorreiterrolle und hohe Erfolgsquote bewegen uns dazu, diesen Eingriff in USA vornehmen zu lassen. Zu unser großen Freude, die mit unendlicher Hoffnung verbunden ist, hat sich Dr. TS Park für eine Selektive Dorsale Rhizotomie (SDR) am St. Louis Children’s Hospital ausgesprochen. Er sieht in mir einen guten Kandidaten für diesen Eingriff mit vielversprechenden Erfolgsaussichten.

After the chances of success through SDR had already been mentioned in Germany, my mother initiated the extensive screening process for treatment by Dr. TS Park. In particular, his minimally invasive approach, extensive experience, medical pioneering role and high success rate convince us to have this procedure carried out in the USA. To our great joy, which is connected with infinite hope, Dr. TS Park has accepted me for SDR at St. Louis Children's Hospital. He sees me as a good candidate for this procedure with promising prospects of success.

Gern möchte ich mich dieser Operation in der zweiten Jahreshälfte 2023 unterziehen, um ausreichend Zeit für Genesung und Rehabilitation vor meinem Schuleintritt im Sommer 2024 einzuplanen. Ich bin auf der Warteliste für die Operation im Oktober / November 2023.

I would like to undergo this surgery in the second half of 2023 to allow enough time for recovery and rehabilitation before starting primary school in the summer of 2024. I am on the waiting list for the surgery in October/November 2023.

Leider handelt es sich bei diesem Eingriff im Ausland um keine Krankenkassenleistung und ist aus privaten Mitteln zu finanzieren. Basierend auf einer vorläufigen Kostenkalkulation muss ich mit einer finanziellen Belastung von € 100.000 (Operation, Rehabilitation, Hilfsmittel, Unterkunft, Verpflegung und Flüge) rechnen.

Unfortunately, this procedure abroad is not covered by health insurance and must be financed from private funds. Based on a preliminary cost calculation, I have to expect a financial burden of €100,000 (surgery, rehabilitation, aids, accommodation, meals and flights).

Den Großteil meiner bisherigen ‘privaten’ Aktivitäten finanziert meine Mutter aus Ihrer beruflichen Selbständigkeit, jedoch stoßen wir insbesondere in diesem Fall an die Grenzen unserer finanziellen Möglichkeiten.
