Critical disabled man needs help

My name is Jose ortiz i am a 33 year old married and father of two kids, facing health problems and possible homelessness. After years of fighting kidney disease, heart disease, and diabetes i feel as though i have come to the end of the rope. I have had 6 major heart attacks and chronic illness, are diminishing most of my literal senses. Due to my diabetes i have had my leg and the majority of my fingers amputated, including loosing my eyes sights, which enables me to provide adequate basic need to my family and a better financial situation.
