Introduction

Siddharth Foundation is a non-profit organization that aims to provide affordable primary care to all individuals and families. We believe that everyone deserves access to quality healthcare, regardless of whether they can qualify for or afford insurance or not.

Problem

By the latest estimates, over 28 million people in the US have no health insurance. In addition, based on research by Kaiser Foundation, between 8-24 million people will lose Medicaid coverage as the programs under the public health emergency expire.

There is a stark contrast in the quality of medical care between insured and uninsured patients. Insured patients typically have a Primary Care Physician (PCP) who manages and coordinates their care, while uninsured patients often rely on whatever facility they have access to during a health emergency. Uninsured patients often lack access to medical records and follow-up care, making it difficult for doctors to provide accurate diagnoses and appropriate treatment.

People without insurance face many challenges when it comes to healthcare, such as:

• No preventive care: They miss out on screenings, check-ups, and vaccinations that can prevent serious health issues and save money in the long run.

• High costs: They have to pay for their medical care out of their own pockets, which can be very expensive and unpredictable. They may have trouble paying medical bills or end up in debt or bankruptcy.

• Limited access: They may have difficulty finding a provider who will accept them or offer them the services they need. They may also wait too long or avoid seeking care due to cost or access barriers, which can worsen their health and increase the risk of emergencies.

• No support: They may not have a regular source of care or a primary care physician who can coordinate their care and provide them with support and guidance. They may also lack information and education about their health conditions and treatment options.

Solution

Siddharth Foundation, in partnership with MyPhysicianPlan, is working on a solution so the uninsured have access to a dedicated board-certified physician for primary care. This will enable the 4C’s of primary care which are essential for good health.



• Contact - Reach out to your Primary Care Provider as the first point of contact, including before visiting the Emergency Room if feasible

• Continuity - Reach out to Primary Care Provider for ongoing and longitudinal care, and maintaining a trusting relationship over time

• Comprehensive - Rely on PCP for addressing the physical, mental, social and environmental needs

• Coordination - Utilize PCP for referrals and coordination of care with different settings and stakeholders

Request for Funding

We are requesting a grant of $100,000 to help us cover the cost of extending primary health care to all through alternative models that are self-sustaining and market-oriented. This funding will allow us to:

• Expand our outreach efforts to reach more people and provide financial assistance to help people afford plans (45-55%)

• Provide development support to continue to build and support the program (30-40%)

• Provide admin support to organizations supporting the program (10-20%)

Impact

The funding from this grant will have a significant impact on the lives of individuals and families currently without insurance. It will allow us to provide them with access to quality healthcare, which can improve their health and well-being. It will also help to reduce the financial burden of healthcare, which can free up resources for other important needs.
