SAVE A LIFE OF A RENAL FAILURE PATIENT.

Hi, my name is Dr. Anthony S. Lewis-Martin, I am the Uncle of Maurice Scott Lewis, Brother of Debra Louise Lewis who passed away in July 2020 from a Heart Attact. I'm trying to assist as best I can to help my nephew get back and forth to Davita Dialysis located at 6225 Atlanta Highway Suite 117, Alpharetta, GA 30004 Telephone Number [phone redacted]. His Social Workername is Jasmine. Maurice is having a rough time with his vision and is in desperate need of assistance from the community to make sure that he gets back and forth to dialysis weekly until he can afford to see again and even gain control of his finances. But in the meantime without him going weekly to Davita he would DIE. I have been doing everything I could do personally to assist to get him to and from multiple appointments, but it's becoming too much for me as I am disabled myself and have a lack of funds to continue to help. I'm afraid for him and scared for him as well.
