Spinal fusion surgery needed for single father

My name is Sean, and I'm a single father of a 16 year old girl. I've been out of work since March due to my back, and I need surgery to fix my back. I have a fracture, degenerative disc disease, spondylosis and spondylolisthesis. I have trouble walking, standing and even sitting as the pain is all over my back and shoots down my right leg.

I've used up all the money I had available in my 401k and my credit cards are maxed out. And I'm currently fighting with Sedgwick who denied my disability claim (despite approving it previously), even though my orthopedic doctor told them I would be out of work. I won't even be able to pay rent this month, so I need some help ASAP. I have insurance, but I'll still need money for deductibles, and will need money for food, bills, and any help I need after the surgery. I don't know if this will cover it but it'll at least help for now.
