Support for Brian Moffett's ALS Legacy and Memorial Costs

We're looking to get my dad home and comfortable with the care and resources he needs.
Earlier this year, my dad was diagnosed with ALS. Since then, he's lost nearly all of his mobility and needs assistance with daily activities, cooking, cleaning, and everything in between.
He's been in a rehab program for the last month where he's learning to preserve any mobility he can. He's currently "voice banking", a program that will allow him to have certain phrases logged so he can communicate when he can no longer speak.
We're looking to bring him home mid-June so he can live out the remainder of his life peacefully, in his upstate NY home.
Recently, my dad has become the poster child for the Death With Dignity Act in NYC. He's advocating for end of life choices, and his story is resonating with voters and legislators around the state.
Help us make my dad's last few months comfortable as we look to bring in private home aids to help him.
