When Travis and Haley Trapp found out they had twins, they were warned the babies’ different sizes could be a cause for concern or could be nothing. After seeing a Fetal Medicine specialist, they were told the twins had TRAP Sequence.
Twin reversed arterial perfusion is a rare, serious complication that affects twins sharing the same placenta. One twin develops normally, while the other is severely malformed, generally missing an upper body, head, arms and a heart. The severely malformed “acardiac” twin lacks the vital organs necessary for survival outside of the womb. The healthy fetus is known as the “pump” twin because it pumps blood to the “acardiac” twin through the umbilical artery of the “acardiac” twin that usually carries blood in the opposite direction. This backward blood flow is known as “reverse arterial perfusion.”
The increased demands on the heart of the “pump” twin increase the risk of serious pregnancy complications. The larger the “acardiac” twin grows, the greater the risks:
- Too much amniotic fluid
- Preterm labor and birth
- Hydrops, threatening the life of the “pump” twin
- Heart failure in the “pump” twin
- Miscarriage
- Stillbirth
- Death of the “pump” twin shortly after birth due to heart failure or other complications
Doctors have been closely monitoring Haley, precious baby Eleanor (the healthy “pump” twin), and Baby B (the “acardiac” twin). Their most recent appointments show increased fluids, indicating that Eleanor is going into heart failure. Haley required an emergency Amniocentesis and will need to fly to Texas for in-utero fetal surgery to stop the blood flow from Eleanor to Baby B. The Trapp Family has already had extra tests, medications, appointments, procedures, and missed work. They now require travel to Texas, continued medical monitoring and procedures, and out-of-state hotel stays.
