Surgery for Parkinsons patient

A 46 year old father of two got diagnosed with Parkinson's disease about 7 years ago. He was initially managed with oral medications with some relief but lately for the last 8 years he has become refractory to medications. He now has progressed to end stage Parkinson's and also has significant side effects from medications which has limited his day to day daily activity. He needs Deep Brain Stimulator (DBS) surgery as a last resort which is quite an invasive and expensive procedure. He only makes Rs. 20,000/ month ( US 200) and unable to afford the surgery.
I am a practicing neurologist in US and I have been managing his medications remotely over last many years. I am originally from Abbottabad, Pakistan and this gentleman was the gatekeeper of the community where we lived.
