Greater Milwaukee Center for Health and Wellness, Inc.                                               www.gmchealth.org  Greater Milwaukee Center for Health and Wellness, Inc., is a non-profit agency of volunteer staff dedicated to providing primary healthcare and social service care coordination to the Greater Milwaukee area, with special attention on vulnerable populations that traditionally experience barriers to accessing healthcare. Our clinical expertise includes Women's Health, HIV, Hepatitis C, Women's and Transgender Health, with our ultimate goal of offering GMC as a primary care medical home for everyone. Within each of the listed areas, resides populations with health conditions and life circumstances requiring additional health education, care coordination and counseling. We Provide: *Primary and Specialty care *HIV/STI counseling, testing and referral *Case Managment *On-site pharmacy and medication management *Health Insurance enrolllment assistance *Referrals to other community partners such as: *Pathfinders *IMPACT 2-1-1 *Milw County Mental Health Complex *Legal assistance agencies *Dental Health providers *Substance abuse treatment facilities Your financial support will help us to continue our mission to serve this vulnerable population and provide the critical access to healthcare needed.
