Help Duchess the Horse

My name is Duchess. I'm a 16 - year-old horse and I enjoy eating grass and getting pats and treats. But my life hasn't always been easy.
I was purchased by someone for his kids, and they didn't know how to ride me properly. They would kick me with spurs and force me to run and run until I couldn't anymore. Eventually, I learned that if I bit and bucked and kicked enough, they would leave me alone. I got the reputation of being a mean old horse, but I didn't care.
Eventually, the man who bought me couldn't afford me anymore, so he left me with the owner of the barn I was staying in. I stayed in my stall all day, watching people come and go without getting much attention. I noticed a new human started coming to the barn, and she would occasionally stop at my stall and talk to me and give me treats. Then one day, she came into my stall and took me out. I didn't know what was going on, but she put me in some cross ties and brushed me. I didn't mind the brushing, but she brought out my saddle and I didn't like that much. I pinned my ears and tried to bite or kick her, but even though she was scared, she and her friends put the saddle on me and she rode me around. It had been a long time since I'd been out of my stall, so I didn't mind the riding. I trotted around and around, even though the human didn't want me to, and enjoyed myself.
I started seeing this human more and more, and she would always take me outside and let me eat grass while she brushed me. She rode me as well, which I didn't like very much, especially when she put her heels against my sides. I thought I was going to get kicked with spurs, so I would buck or kick.
Through love and discipline, I learned that this human meant me no harm. I came to like her a lot, and she even bought me!
Just recently, she moved me to a new barn where I can graze outside and be around other horses. This was a big change for me, because everyone said that I was a mean and aggressive horse, and that I wasn't safe around other horses. I proved them wrong, though! I mostly keep to myself, but I've befriended lots of horses at the new barn. I'm living a much better life than I was, thanks to this human. I really like this human, and she loves me with all her heart.
But I know that my human isn't very well.
She tries her best to take care of me, but she gets worn out easily. Sometimes, she'll cry with me, even though there's nothing wrong. She tells me that she loves me, but she's worried for my health because she doesn't make very much money.
Since my move from the old barn to the new one, I've been having trouble with my hooves. I spent a lot of time in my stall at the old barn, and my hooves have become soft and are peeling and cracking. It's really painful for me, and my human can't visit me frequently as she has no car and I live far away from her. She's doing everything she can, but it isn't enough.
She's considered selling me since she doesn't make much money, but not many people want a horse like me. I can't do any competitive riding because it hurts my legs, and I'm too finicky for an inexperienced rider or for lessons. She also loves me to death, and I think that giving me away would make us both extremely heartbroken. This is why I need your help.
In addition to my hooves, I also cost a lot for check ups and vaccines, and my teeth need to be checked often as well. My human is doing everything she can, but it just isn't enough.
With the money earned from this GoFundMe, my human is going to get my hooves fixed and make sure that I get all my check ups. All of it will be going to helping me live a happy and comfortable life with the human who's tried so hard when no one else cared. I don't want to lose my human, and she doesn't want to lose me, either.
