Medical Treament for Dog Hit by Car

On October 22nd while working in Columbus, Georgia I witnessed a man driving a white 1998 Ford Explorer hit this poor little dog on Fairview Drive. The driver didn't slow down a bit, he sped up after the dog was completely run over to avoid any confrontation or consequences. I immediately thought that this dog was dead, fortunately he was lucky enough to roll under the truck and not under any of the tires. He then took off running into the woods, visibly injured, clearly experiencing the shock and confusion of being hit by a truck. 
I just turned the corner of an opening in the fence when I watched this happen, I was quickly thrown into fight or flight mode because of distress of the situation. I was angry at the driver, but more overwhelmingly concerned for the little dog. I grabbed a pair of loppers I had in my truck bed and started tearing through the woods I saw him run into. I ended up going back into the woods a good [phone redacted] yards until I came across a number of abandoned buildings, after going through each one I found the little guy. He was badly injured and in rough shape, I didn't want to carry him because I was afraid to hurt him any further so I drove my truck into the woods and loaded him up. 
I took him to the Companion Animal Hospital in Phenix City, Alabama. The dog had a testicle hanging out, road rash on all 4 legs and under belly, and bad cuts and bruises all over. Amazingly he had no broken bones.
The Vets preformed an emergency neuter, stitched up his cuts and treated his other wounds. We ended up also removing a tumor he had on his leg and updating all his shots. I ended up covering the $870.46 Veterinarian bill myself because I couldn't let myself walk away from medical treatment for this animal. 

The reason I am posting this is because Alexander needs more medical treatment. After running a test on his blood work the vets found that Alex has heart worms, I received estimates on the treatment plan yesterday while the vets were removing his stitches.

Heart-worm treatment plans are done in 3 Phases, each phase has a separate estimate. 

Phase 1 is $323.83, Phase 2 is $406.51, and Phase 3 is $539.67.

All together the treatment totals $1270.01. 

I am trying to raise the $1270.01 here on gofundme so I can provide this dog with a better quality of life.
