Moving and medical bills

My name is Johanna, and I'm a disabled mother of two.  I suffer from lupus and fibromyilga so severe that I can't work anymore and have been on disability for the last few years.  My husband and I are seperating and now I have to move to a new, low income apartment which means breaking the lease I have here or getting evicted.

I have my disabilities and need over 10k in dental work done, plus the fees to be able to move me and my daughter into a new apartment and furnish it and make sure we have some food.

Any help you could add would be a large blessing for us.
